Job Description

We are seeking a qualified Home Health Physical Therapist to provide direct patient care in a home health setting. This role involves assessing patient needs, developing individualized treatment plans, and implementing therapeutic interventions in collaboration with patients and their families.

Responsibilities

• Conduct patient evaluations and develop tailored treatment plans.
• Deliver one-on-one therapy to patients in their homes.
• Educate patients and their families on home exercise programs and rehabilitation techniques.
• Maintain accurate and updated patient records using OASIS and EPIC charting systems.
• Coordinate with interdisciplinary teams to ensure comprehensive patient care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience required.
• BCLS certification required.
• Oasis experience preferred.
• Reliable transportation is a must for patient visits.
• COVID primary series vaccination and one booster required.
