Job Description

We are seeking a dedicated Home Health Physical Therapist to provide quality care in the Medford, OR area. The ideal candidate will possess a minimum of 1 year of skilled home health experience. This position requires the ability to effectively manage patient care with a standard productivity of 30 points, with up to 6 patients visited per day.

Responsibilities

• Conduct patient evaluations and develop treatment plans.
• Implement therapy interventions according to the therapeutic plan.
• Educate patients and families about therapeutic processes.
• Utilize the Oasis system for patient documentation and care planning.
• Drive to various patient locations efficiently, minimizing time spent commuting.

Required Experience / Certifications / Licensure

• A minimum of 1 year of skilled home health experience is required.
• Current BCLS certification is mandatory.
• Experience with Oasis documentation is required.
• Active State licensure as a Physical Therapist.
• Traveler must have reliable transportation.
