Day shift: 8-hour shifts from 08:00 to 16:30. This facility cannot accommodate 4x10s or 3x12s.

Job Description

The Home Health Physical Therapist will provide direct patient care in a home health setting. The position is open to travelers with no prior home health experience, allowing first-time travelers the opportunity to expand their skill set in this specialty.

Responsibilities

• Deliver one-on-one patient care with a focus on rehabilitation and recovery.
• Visit up to 6 patients per day, adhering to a standard productivity point system.
• Administer evaluations, intervention plans, and adjust treatment based on patient progress.
• Documentation using Oasis and Epic charting systems.

Required Experience / Certifications / Licensure

• BCLS certification required.
• Requires own transportation; some maximum travel radius may extend to 50+ miles.
• COVID vaccination series and booster required.
• Oasis experience preferred; travelers without it are still encouraged to apply.
