Job Description

The Home Health Physical Therapist is responsible for providing direct patient care in a home health setting, conducting evaluations, and developing treatment plans tailored to individual patient needs.

Responsibilities

• Conduct patient evaluations and develop individualized treatment plans.
• Implement therapeutic exercises and interventions.
• Monitor patient progress and adjust treatment plans as necessary.
• Provide education to patients and their families on home exercise programs and essential care strategies.
• Maintain accurate and timely documentation using the EPIC charting system and Oasis.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience.
• Current BCLS certification is required.
• Oasis experience is mandatory.
• Must have a valid Physical Therapist license in the state of Oregon.
