Day shift, 5x8-Hour (08:00 - 16:30). This facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking a dedicated Home Health Physical Therapist to join our team in East Portland, OR, and surrounding areas. The ideal candidate will be responsible for providing high-quality therapy services to patients in their homes, with an emphasis on individualized care and patient satisfaction. This role requires strong clinical skills and the ability to work independently in a patient's home environment.

Responsibilities

• Assess and evaluate patients to determine their therapy needs.
• Develop and implement individualized treatment plans aimed at improving patient mobility and quality of life.
• Provide therapy interventions including therapeutic exercises, manual therapy, and the use of specialized equipment.
• Educate patients and their families on treatment plans and home exercise programs.
• Document patient progress and maintain accurate records using EPIC and OASIS systems.
• Coordinate with other healthcare professionals to ensure comprehensive patient care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience is required.
• BLS certification is required.
• OASIS experience is required.
• Must have a valid state licensure to practice as a Physical Therapist.
