Day shift: 8-hour days (08:00 - 16:30). This facility cannot accommodate 4x10s or 3x12s.

Job Description

The Home Health Physical Therapist Assistant will provide direct patient care in the home setting, focusing on rehabilitation and recovery. The position entails visiting patients, implementing treatment plans, and supporting physical therapy interventions as prescribed by the supervising therapist.

Responsibilities

• Deliver one-on-one patient care in home environments, adhering to treatment protocols.
• Assist patients with mobility exercises and the use of therapeutic equipment.
• Document patient progress and report findings to the supervising therapist.
• Ensure patient safety and comfort during therapy sessions.
• Manage a case load of up to 6 patients daily while maintaining productivity standards.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience is required.
• Current Basic Life Support (BCLS) certification is required.
• Possession of a valid state Physical Therapist Assistant license.
• Experience with charting systems such as EPIC is preferred.
