Job Description

Seeking a qualified Home Health Physical Therapist with licensure in both Oregon and Washington to provide care in the East Portland and Clark County areas. The position involves comprehensive patient evaluations and the creation and implementation of individualized treatment plans.

Responsibilities

• Conduct patient evaluations and develop treatment plans tailored to individual needs.
• Provide one-on-one therapy in home health settings.
• Ongoing assessment and modification of treatment plans as necessary.
• Document patient progress in the designated charting systems (EPIC Remote Client for Home Health and Rover).
• Manage a caseload of up to six patients per day while ensuring adequate documentation and productivity.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience required.
• Current BCLS certification is required.
• Active PT licensure in Oregon and Washington required.
• Experience with OASIS documentation is necessary.
• Ability to transport oneself between patient visits in a personal vehicle is essential.
