Job Description

We are seeking a dedicated Home Health Physical Therapist Assistant (PTA) to provide one-on-one care to patients in their homes. The ideal candidate will have experience in skilled home health settings and be able to manage a varied caseload while maintaining productivity standards.

Responsibilities

• Deliver exceptional home health care to patients, focusing on rehabilitation and recovery.
• Assist in evaluating patient needs and developing individualized treatment plans in collaboration with physical therapists.
• Support patients in achieving their rehabilitation goals and improving their quality of life.
• Document patient progress and maintain accurate records using the charting systems provided.
• Manage a productive schedule and ensure timely visits to up to six patients per day.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience is required.
• Basic Cardiac Life Support (BCLS) certification is required.
• Must have reliable transportation and a valid driver's license.
• Primary series COVID vaccination plus one booster is required.
