Day shift, 5x8 Hour Schedule (08:00 - 16:30). Note: This facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking a skilled Home Health Physical Therapy Assistant (PTA) to provide direct patient care in various home settings. The PTA will work with a diverse patient population, supporting their rehabilitation and recovery needs.

Responsibilities

• Provide one-on-one patient care and assist in rehabilitation exercises.
• May visit up to 6 patients per day, adhering to a productivity standard of 30 points.
• Utilize EPIC Remote Client for documentation and chart management.
• Ensure high-quality patient care in compliance with safety regulations.

Required Experience & Certifications

• Minimum of 1 year of skilled home health experience required.
• BCLS certification is required.
• Reliable transportation is necessary for travel between patients’ homes.
• COVID-19 vaccination (primary series and one booster) is required.
