Job Description

The Outpatient Physical Therapist will evaluate all assigned patients, developing, implementing, and modifying treatment plans in accordance with patient needs, goals, and physician directions. This role includes the delegation and direction of all discipline-specific therapy services, ensuring the design of functional treatment programs for effective community reintegration. The therapist will serve as an educator for patients, families, students, coworkers, and themselves, adhering to professional standards and organizational compliance.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals focused on timely discharge.
• Document medical records in a clear, concise, and timely manner.
• Ensure care is provided under signed, valid physician orders and within insurance approval.
• Participate in student programs as assigned, providing supervision and evaluation.
• Create and actively pursue a personalized professional development plan.
• Adhere to discipline-specific State Practice Acts and maintain licensure.
• Understand and participate in organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
