Licensed Practical Nurse (LPN) - Night Shifts

Schedule

Night Shifts - 12 hour shifts. Taking call is required 2 shifts per 6 weeks with potential for overtime.

Job Description

The LPN providing care to the patient population(s) is responsible for assessing, planning, implementing, and coordinating patient care in Skilled Nursing/Extended Care settings.

Responsibilities

• Assess and evaluate patient needs and care requirements.
• Plan and implement nursing care as per established clinical guidelines and protocols.
• Coordinate with healthcare team members to ensure optimal patient care.
• Administer medications and perform treatments as prescribed.
• Document patient progress and communicate effectively with the team.

Required Experience / Certifications / Licensure

• Current LPN licensure in the state of Colorado.
• Previous experience in Skilled Nursing or Extended Care preferred.
