Day Shift: Monday to Friday, 8:00 AM - 4:30 PM

Job Description

The Travel RN - Case Manager will be responsible for comprehensive case management services including assessment and discharge planning. The position requires collaboration with a team of healthcare professionals in a fast-paced and dynamic environment.

Responsibilities

• Conduct case management assessments.
• Develop and implement discharge plans.
• Coordinate care for patients transitioning within the healthcare system.
• Collaborate with physicians, mid-levels, and other staff for optimal patient outcomes.

Required Experience / Certifications / Licensure

• Minimum of 1-year current acute care case management RN experience.
• Active Kansas RN licensure or licensed social worker.
• Current BLS certification.
• Experience with Meditech and Midas is strongly preferred.
