Job Description

The Wound, Ostomy and Continence Nurse provides patient care based on established nursing guidelines and applicable standards of care. This role serves as a liaison for wound, ostomy, and continence care concerns and includes educating team members while participating in assigned committees.

Responsibilities

• Assess the physical, psychological, and social needs of assigned patients, initiating data collection and assessment.
• Contribute to the development of the plan of care for each assigned patient in collaboration with the interdisciplinary care team.
• Initiate and implement nursing interventions, medical orders, and related therapies.
• Collaborate with the healthcare team to educate patients and their support systems.
• Analyze, modify, and implement strategies to progress patients through their plan of care.
• Document patient records reflecting all care provided and responses according to professional standards.
• Monitor equipment and perform routine and complex treatments, including ostomy care, wound care, and incontinence care.
• Comply with mandatory reporting, legal consent, and other requirements to preserve patient rights and ensure the appropriateness of care.
• Ensure a clean, safe, and secure environment by following safety policies and procedures.
• Handle biomedical, pharmaceutical, hazardous, and universal wastes in accordance with regulatory standards.

Required Experience / Certifications / Licensure

• Inpatient acute wound care experience is required.
• Experience in continence care and ostomy care is a must.
