Job Description

We are seeking a dedicated Home Health Physical Therapist Assistant to join our team. The successful candidate will deliver one-on-one patient care, providing essential support to patients in the home health setting.

Responsibilities

• Manage care for up to 6 patients per day with a focus on high-quality service.
• Assist patients with rehabilitation exercises under the supervision of a licensed physical therapist.
• Document patient progress and adjust treatment plans as needed using EPIC charting software and Rover application.
• Maintain effective communication with patients, families, and healthcare team members.

Required Experience / Certifications / Licensure

• 1 year of skilled home health experience required.
• Current BCLS certification needed.
• Reliable transportation is required for patient visits.
• Must be fully vaccinated against COVID-19, including a booster.
