Community Health LPN

Job Description

The Community Health LPN provides and documents nursing care for the sick and injured in a home health setting. This role promotes, maintains, or restores health to prevent illness and effects rehabilitation by evaluating individuals' and families' responses to interventions.

Responsibilities

• Collaborate with healthcare providers and community representatives to coordinate care at home.
• Utilize time management, communication, planning, delegating, and problem-solving skills to provide care.
• Exercise independent judgment and demonstrate critical thinking skills in emergency settings.
• Maintain emotional stability and confidentiality of patient information.
• Drive and navigate in various weather conditions for home visits.
• Be flexible and willing to accept on-call responsibilities as needed.

Required Experience / Certifications / Licensure

• Minimum of one year of acute nursing care experience; hospital experience preferred.
• Experience in home health and proficiency with OASIS is highly preferred.
• Certified in Home Health and possess Infusion Therapy experience (preferred).
• Current Massachusetts LPN license must be uploaded with profile.
• Active certification in BLS from American Heart Association or American Red Cross.
• Must possess a valid driver’s license and vehicle for transportation.

Schedule

This position is scheduled for 8:00 AM to 4:30 PM, with an 8-hour workday requirement.
