Registered Nurse Non-Certified Case Manager

Job Description

The Registered Nurse Non-Certified Case Manager works collaboratively with physicians and multidisciplinary team members to develop individualized care plans for assigned patients. The role is crucial in ensuring that patients progress towards desired health outcomes through continuous assessments and record monitoring. This position actively resolves barriers to effective patient care and is deeply involved in the discharge planning process.

Responsibilities

• Conduct comprehensive assessments of patients within regulated timelines to determine eligibility for case management.
• Develop and implement case management plans in collaboration with patients, caregivers, and healthcare professionals.
• Monitor and evaluate the effectiveness of care plans, documenting interventions and goal achievement.
• Maintain a regular case load with ongoing patient outreach and management.
• Facilitate the integration of services, including behavioral health care, to improve continuity of care.

Required Experience / Certifications / Licensure

• Current Montana Licensure as a Registered Nurse.
• American Heart Association Basic Life Support (BLS) certification.
• Minimum of 2 years of nursing experience with a focus on utilization review.
• Preferred: Bachelor’s Degree in Nursing.
• Preferred: 2 years of acute care hospital experience as a case manager.

Schedule

This is a Day Shift position from 7:30 AM to 4:30 PM, with weekend rotations required.

Assignment Duration

This is a 13-week contract position.
