Job Description

The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans in accordance with patient needs, goals, and physician direction. This role includes the delegation and direction of all discipline-specific therapy services for those assigned patients. The therapist ensures the design of functional treatment programs allowing for effective return to community activities and serves as an educator for patients, families, students, coworkers, and themselves.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop individualized treatment plans and goals.
• Document care provided in compliance with state, federal, and organizational guidelines.
• Ensure all care is provided under signed, valid physician orders.
• Participate in student programs and provide guidance as needed.
• Create a professional development plan and pursue continuing education opportunities.
• Adhere to discipline-specific State Practice Act and maintain professional licensure.
• Contribute to the organization’s performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ years of outpatient clinical experience.
• Current CPR certification.
• Active and applicable license for the state where work will be performed.
