Registered Nurse - Case Management

Schedule

Day shift available: 5x8 hours (08:30 - 17:00), Monday to Friday. No exceptions and no call required.

Job Description

The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The overall goal is to enhance the quality of patient management and satisfaction, promote continuity of care, and ensure cost-effectiveness through the integration of case management, utilization review, and discharge planning. The Case Manager must be highly organized, detail-oriented, adaptable to frequent change, and compliant with regulatory and departmental guidelines and policies.

Responsibilities

• Conduct comprehensive assessments for patient needs.
• Develop and implement ongoing care plans.
• Coordinate with interdisciplinary teams to facilitate discharge planning.
• Monitor patient progress and adhere to regulatory standards.
• Utilize tools such as EPIC for case management and documentation.

Required Experience / Certifications / Licensure

• Minimum 4 years of experience as a Registered Nurse with at least 2 years in a healthcare environment.
• Must have acute hospital experience.
• BLS certification required; ACM/CCM preferred.
• Must be licensed to practice as a Registered Nurse in the state of North Carolina.
• EPIC experience required.
