This is a 13-week contract position.

Schedule

Monday to Friday, 8 AM to 4 PM with 2-3 weekend days required.

Job Description

The Physical Therapist will actively monitor and amend patient visit schedules to maximize productivity and meet patient needs. This role involves performing comprehensive assessments, developing plans of care, coordinating patient care, supervising clinical activities, and completing necessary paperwork and documentation. The Physical Therapist will also participate in training programs and engage in community marketing efforts related to rehabilitation services.

Responsibilities

• Actively monitor and amend patient schedules as needed.
• Perform comprehensive patient assessments in accordance with regulations and standards of care.
• Develop and implement plans of care that promote functional independence.
• Provide skilled physical therapy treatment and document progress on each visit.
• Coordinate care with other clinical team members to ensure optimal patient care.
• Supervise clinical activities of PTAs, PT students, and home care aides.
• Complete required paperwork to ensure accurate documentation and billing processes.
• Participate in in-service training programs for various stakeholders.
• Maintain personal knowledge and promote standards in physical therapy practice.
• Engage in the marketing of rehabilitation services and community outreach.
• Conduct various patient-related health assessments and procedures.
• Coordinate safe patient discharges and transfers, ensuring proper documentation.
• Participate in peer review audits and provide constructive feedback.

Required Experience / Certifications / Licensure

Licensed Physical Therapist with relevant experience in home care settings preferred.
