Monday - Friday, daytime shifts (8-hour shifts, 5 days a week)

Job Description

We are seeking a dedicated Home Hospice Nurse to provide compassionate care to patients in the San Bernardino area. This role involves delivering nursing care and support to patients in their homes, ensuring their comfort and dignity. The ideal candidate will possess hospice experience and be capable of managing multiple patient visits efficiently.

Responsibilities

• Provide direct patient care in a home setting.
• Conduct patient assessments and develop care plans.
• Manage medication administration and wound care.
• Educate patients and families on care processes and support.
• Document patient progress and communicate with the healthcare team.
• Adhere to productivity expectations of 24-30 visits per week.

Required Experience / Certifications / Licensure

• Hospice experience required.
• Experience with HomeCare HomeBase (HCHB) preferred; candidates may be considered without HCHB experience at a reduced rate for the first two weeks if training is provided.
• Valid driver’s license and auto insurance required.
