Day shift, Monday to Friday from 08:00 AM to 04:30 PM

Job Description

As a Registered Nurse in Home Health, you will provide essential nursing care to patients primarily in York and Adams counties. Your role will involve assessment, planning, and delivering patient care in a home environment, ensuring comfort and support to patients, including those recuperating from COVID-19.

Responsibilities

• Conduct patient assessments and develop care plans based on individual needs.
• Administer medications and treatments in accordance with established protocols.
• Perform OASIS documentation to ensure compliance and continuity of care.
• Educate patients and families regarding health management and resources.
• Coordinate care with interdisciplinary team members and report significant changes in patient conditions.

Required Experience / Certifications / Licensure

• Minimum of 1 year of nursing experience is required.
• OASIS documentation skills preferred but not mandatory.
• Current RN license in Pennsylvania is required.
• COVID-19 and flu vaccinations are mandatory, with room for medical or religious exemptions.
