LPN - Long Term Care

Schedule

This is a 13-week contract position with day shifts from 6:30 AM to 2:30 PM. Every other weekend is required. Guaranteed 37.5 hours per week.

Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) for a long-term care facility. The successful candidate will provide quality nursing care to patients in a 40-bed unit, which includes populations with multiple sclerosis, orthopedic issues, dementia, and memory care.

Responsibilities

• Provide nursing care and support to patients across various units.
• Administer medications and perform scheduled treatments.
• Monitor patient conditions and report changes to the healthcare team.
• Assist with activities of daily living and ensure patient comfort.
• Utilize electronic medical records (EMR) with PointClick Care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of relevant nursing experience.
• Current New York State LPN license.
• Basic Life Support (BLS) certification.
