Social Worker - Case Manager / Discharge Planner

This is a 13-week contract position.

Schedule

Day shift, 5x8-Hour schedule from 08:00 AM to 04:30 PM, with weekend requirements of every other weekend.

Job Description

We are seeking a dedicated Social Worker to join our care management department. The selected candidate will be responsible for providing case management and discharge planning for a diverse patient population, ensuring safe and effective transitions of care.

Responsibilities

• Conduct initial assessments within 24 hours of patient admission.
• Coordinate discharge plans for patients, ensuring all post-acute care needs are met.
• Work closely with the medical team and insurance case managers to facilitate discharges to the lowest level of care.
• Manage a caseload of 20-23 patients, providing care coordination and support.
• Educate patients and caregivers, reinforcing individualized care plans.
• Document care provided and collaborate with interdisciplinary teams to enhance patient outcomes.

Required Experience / Certifications / Licensure

• Minimum of 1 year of acute care experience required.
• CA state license required; pending licenses accepted if licensed upon start date.
• Certifications required: BLS, and either LCSW or MSW.
• Experience with case management and discharge planning in an acute care setting is essential.
• Knowledge of Epic charting system preferred.
