Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with patient needs, goals, and physician direction. Responsible for delegation and direction of all discipline-specific therapy services for assigned patients, ensuring functional treatment programs are designed to facilitate effective community reintegration. The therapist serves as an educator for patients, families, students, and coworkers while upholding the organization's Mission Statement and Core Values.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop and implement appropriate treatment plans and goals.
• Document patient care accurately and timely, in compliance with state, federal, and organizational guidelines.
• Participate in student training programs, providing supervision and evaluations.
• Create an individualized professional development plan and pursue continuing education.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing.
• Participate in performance improvement initiatives and contribute feedback for maximizing outcomes.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited program.
• 6 months to 1 year of outpatient clinical experience.
• CPR Certification.
• Active and applicable state licensure for Physical Therapy.
• Prior experience with Select Medical or similar agencies is highly desired.
