Outpatient Physical Therapist
Job Description
The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient's needs, goals, and physician direction. This role is responsible for delegation and direction of all discipline-specific therapy services for assigned patients while ensuring the design of functional treatment programs aimed at effective return to community activities. The therapist serves as an educator for patients, families, students, coworkers, and self, and performs other professional duties associated with patient care and program delivery.
Responsibilities
- Complete comprehensive evaluations of assigned patients.
- Utilize clinical reasoning to develop appropriate treatment plans and goals, focusing on patient discharge within an established length of stay.
- Document care in a clear, concise, accurate, and timely manner, ensuring compliance with state and federal guidelines.
- Participate in student programs as assigned, providing a positive learning environment.
- Create and actively pursue an individualized professional development plan.
- Understand the organization’s performance improvement methodologies and follow through with action plans.
Required Experience / Certifications / Licensure
- Degree in Physical Therapy from an accredited school.
- 1+ year of outpatient clinical experience.
- CPR certification.
- Maintains professional licensure in good standing and completes all state-mandated continuing education requirements.
Schedule
This position operates on a 5x8.0 schedule.
Assignment Duration
This is a 13-week contract position.
