Monday to Friday, 8:00 AM - 4:30 PM with rotating weekends.

Job Description

The Registered Nurse in Case Management will be responsible for patient assessments, discharge planning, and coordinating care for patients transitioning from hospital to home or other care settings.

Responsibilities

• Conduct patient assessments and develop discharge plans.
• Coordinate with healthcare teams, including social workers and case management assistants.
• Utilize electronic documentation systems such as Meditech, Midas, and Wellsky.
• Ensure compliance with hospital policies and patient care standards.

Required Experience / Certifications / Licensure

• Recent experience in acute care discharge planning is required.
• A valid RN license in the state of Missouri is mandatory.
