Outpatient Physical Therapist

Schedule

Day shift, 5x8 hours (08:00 AM - 05:00 PM), no weekends or holidays required.

Job Description

We are seeking a skilled Outpatient Physical Therapist with a minimum of 1 year of experience. The ideal candidate will possess a North Carolina PT license and be proficient in utilizing Epic EMR. This role involves providing therapy interventions, evaluations, and developing treatment plans for various patient diagnoses including orthopedic, neuro (stroke, Parkinson's, MS), geriatric conditions (debilitating falls, dementia), post-surgical rehabilitation, and chronic conditions such as COPD.

Responsibilities

• Conduct assessments and evaluations of patients.
• Develop and implement individualized treatment plans.
• Utilize adaptive devices and assistive technologies to assist patients in regaining mobility and independence.
• Monitor patient progress and make necessary adjustments to the treatment plan.
• Educate patients and their families about care plans and recovery processes.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience as a Physical Therapist.
• Active North Carolina PT license required.
• Experience with Epic EMR preferred.
• Current BLS certification.
