Outpatient Physical Therapist

Assignment Duration

This is a 13-week contract position.

Schedule

5x8.0 schedule, Monday through Friday, with approximately 32-40 hours per week.

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient's needs, goals, and physician direction. Responsibilities include the delegation and direction of all discipline-specific therapy services for assigned patients to ensure the design of functional treatment programs that allow for effective community reintegration. The therapist will also serve as an educator for patients, families, students, and coworkers.

Responsibilities

• Conduct comprehensive evaluations of assigned patients
• Develop appropriate treatment plans and goals focused on patient discharge
• Document care in a clear, concise, accurate, and timely manner
• Participate in student programs, providing supervision and evaluations
• Create and implement a professional development plan with management
• Adhere to ethical standards and maintain professional licensure
• Maintain compliance with organizational performance improvement initiatives

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution
• 1+ year of outpatient clinical experience
• Active CPR certification
• Current state licensure as a Physical Therapist
