Patient Centered Physical Therapist

Job Description

A rural healthcare organization is seeking a dedicated Patient Centered Physical Therapist to join their team. The ideal candidate will demonstrate a passion for providing high-quality rehabilitation services while collaborating with a multidisciplinary team to enhance patient care.

Responsibilities

• Conduct evaluations and assessments of patients to determine appropriate treatment plans.
• Implement and monitor individualized therapy programs aimed at achieving rehabilitation goals.
• Educate patients and their families about treatment processes, progress, and home therapy exercises.
• Collaborate with other healthcare professionals to optimize patient outcomes.
• Document patient progress and modify treatment plans as needed.

Schedule

This position offers a standard schedule from 8 AM to 5 PM, ensuring a balanced work-life experience.

Location

This position is situated in a rural healthcare setting, providing a unique opportunity to serve a diverse patient population.

Assignment Duration

This is a 13-week contract position.

Required Experience / Certifications / Licensure

• Current state licensure as a Physical Therapist.
• Experience in a patient-centered therapeutic environment is preferred.
