Nights, 6:30 PM - 7:00 AM, Every other weekend required.

Job Description

We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our Transitional Care team. In this role, you will work closely with patients to provide essential care and support as they recover and prepare for discharge.

Responsibilities

• Assist patients with daily living activities (ADLs) to promote independence and comfort.
• Monitor and record vital signs, including blood glucose checks.
• Support patient mobility and ensure safety during transfers.
• Collaborate with healthcare team members to provide patient-centered care.
• Utilize specialized equipment as needed for patient care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience as a CNA in a transitional care, LTC/SNF, or related setting.
• Current Maine State CNA license.
• Basic Life Support (BLS) certification.
• Experience with glucometer usage and vital signs monitoring.
