Monday to Friday, 8-hour shifts from 8 AM to 5 PM.

Job Description

We are seeking a compassionate and dedicated Home Hospice Registered Nurse to provide high-quality patient care in a home setting. The ideal candidate will be proactive in managing patient needs and will have experience in hospice care.

Responsibilities

• Provide comprehensive nursing care to hospice patients in their homes.
• Manage medication administration, patient assessments, and care planning.
• Coordinate with interdisciplinary team members for holistic patient management.
• Document patient care activities and report changes in patient condition.
• Ensure compliance with safety and health regulations.

Required Experience / Certifications / Licensure

• Registered Nurse (RN) License required.
• Previous hospice experience is essential.
• Experience with HomeCare HomeBase preferred; training may be available for qualified candidates without prior experience.
• Must have valid Driver's License and Auto Insurance.
