Job Description

We are seeking a compassionate and dedicated Hospice Registered Nurse (RN) to provide high-quality care to patients in their homes. The successful candidate will join our team and be responsible for delivering direct patient care, working closely with patients and their families to ensure comfort and quality of life.

Responsibilities

• Provide holistic nursing care to hospice patients and their families.
• Conduct intake assessments and develop care plans.
• Administer medications and manage symptoms effectively.
• Educate patients and families on end-of-life care processes.
• Coordinate with interdisciplinary teams to optimize patient outcomes.
• Document patient care and charting using HCHB and Epic systems.

Required Experience / Certifications / Licensure

• 1 year of skilled hospice nursing experience required.
• Experience with Home Care Home Base (HCHB) is mandatory.
• Current BLS certification.
• Valid Washington State or Compact RN license is required at the time of submission.
• Must have reliable transportation for patient visits.
