Job Description

The Home Health Registered Nurse (RN) will provide quality patient care in the home setting, manage and coordinate patient care, and conduct thorough assessments for patients as required. The role includes high volume documentation and assessment using OASIS standards.

Responsibilities

• Provide nursing care to patients in their homes
• Conduct comprehensive assessments and develop care plans
• Complete required documentation in a timely manner
• Manage a caseload of 5-6 patients per day
• Occasionally perform weekend shifts as required

Required Experience / Certifications / Licensure

• New Jersey RN license is required
• Minimum of 2 years of home health experience
• OASIS experience is mandatory
• Must be a local candidate or a true traveler if commuting over 40 minutes
