Job Description

As an Outpatient Physical Therapist, you will evaluate all assigned patients and develop, implement, and modify treatment plans according to patients' needs, goals, and physician directions. You will be responsible for delegating and directing all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate an effective return to community activities. This role also includes educating patients, families, students, and coworkers. You are expected to uphold the organization's Mission Statement and Core Values while ensuring compliance with the Code of Conduct and maintaining confidentiality.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop treatment plans and goals to progress patients toward discharge.
• Complete medical record documentation accurately and in a timely manner.
• Ensure care is provided under signed, valid physician orders and within insurance approvals.
• Participate in student programs, providing a positive learning environment with appropriate supervision.
• Actively pursue professional development through continuing education and professional activities.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing.
• Understand the organization’s performance improvement methodology and participate in quality improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an Accredited School.
• Outpatient clinical experience of 6 months to 1+ year.
• CPR certification required.
