Location

San Jose, CA and surrounding areas including Santa Clara, Sunnyvale, Cupertino, Milpitas, Mt. View, Campbell, Los Altos, Saratoga, and Los Gatos

Assignment Duration

This is a 13-week contract position.

Schedule

Day shift with potential weekend coverage; additional travel outside the primary areas may be required.

Job Description

CVHCare is seeking a qualified Physical Therapist to provide essential services in a home health setting. The ideal candidate must demonstrate a commitment to delivering high-quality patient care and must be flexible to accommodate varying patient needs within the assigned territories.

Responsibilities

• Conduct comprehensive evaluations of patients using clinical skills and judgment.
• Create individualized treatment plans based on patient assessments.
• Implement therapeutic interventions and monitor patient progress.
• Document patient care and progress in accordance with company policies and federal regulations.
• Coordinate with healthcare teams to enhance patient outcomes and ensure continuity of care.
• Be prepared to treat patients with COVID-19 and those under investigation for COVID-19.

Required Experience / Certifications / Licensure

• Current California Physical Therapist License.
• Basic Life Support (BLS) certification.
• Valid driver's license, auto insurance, and vehicle registration.
• Annual TB test, MMR, Varicella, Hepatitis B, Tdap (10 years), and annual flu vaccination.
• Pass a drug screening and provide a physical exam.
