Job Description

We are seeking a dedicated Physical Therapist to join our team. The ideal candidate will primarily work in outpatient settings while also covering some inpatient and skilled nursing facility (SNF) cases. This role requires a focus on patient care, rehabilitation, and the promotion of health.

Responsibilities

• Provide physical therapy services to patients in outpatient, acute care, and long-term care settings.
• Develop and implement treatment plans based on patient evaluations.
• Educate patients and their families about rehabilitation and home exercise programs.
• Maintain accurate and up-to-date patient records and documentation.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience as a Physical Therapist.
• Current license to practice as a Physical Therapist; pending license accepted.
• Basic Life Support (BLS) certification required.
