Monday - Friday, 8:00 AM to 4:30 PM, with on-call duties during evenings and weekends.

Job Description

We are seeking a dedicated and experienced Home Health RN to provide care to patients in Berkeley, Jefferson, and Morgan counties. The RN will be responsible for patient assessments, education on medications and disease processes, and caregiver training upon discharge. The role includes routine home health visits, as well as addressing urgent needs for patients discharging from hospitals.

Responsibilities

• Conduct assessments and provide education to patients and caregivers.
• Create and implement care plans based on patient needs.
• Manage IV infusions and wound care as necessary.
• Monitor patient progress and document care provided using EPIC charting system.
• Participate in an on-call rotation for after-hours needs.

Required Experience / Certifications / Licensure

• Licensed as a Registered Nurse in West Virginia.
• Experience in home health care is required.
• BLS certification (American Heart Association).
• Ability to manage varying patient conditions, including post-COVID pneumonia, CHF exacerbation, COPD exacerbation, wound care, and IV antibiotics.
