Monday through Friday, 8 am to 4:30 pm.

Job Description

The Physical Therapist will provide home health services to patients in various counties. The therapist will have a manageable caseload of 5-6 patients per day, focusing on a range of diagnoses including general ortho issues, generalized weakness, strengthening, and cardiac rehab.

Responsibilities

• Conduct initial assessments and develop individualized treatment plans.
• Provide ongoing therapy and progress evaluations for patients in a home setting.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Utilize the Epic system for documentation and patient management.
• Participate in staff meetings and professional development activities.

Required Experience / Certifications / Licensure

• Valid Physical Therapist license to practice in West Virginia.
• Basic Life Support (BLS) certification required.
• Experience or willingness to learn the Epic documentation system.
