12-hour evening shifts (11a-11p) with the option of 8-hour shifts available.

Job Description

Join a well-established long-term care facility that focuses on providing skilled nursing assistance, rehabilitative therapies, and social activities to residents. Our goal is to create an environment that encourages residents to thrive and remain socially active.

Responsibilities

• Provide nursing care to residents, ensuring their comfort and well-being.
• Assist in the implementation of patient care plans.
• Monitor and document patients' health status and report changes.
• Collaborate with healthcare team members to optimize patient care.

Required Experience / Certifications / Licensure

• 1-3 years of nursing experience required, with preference for SNF experience.
• Current NY state nursing license required.
• BLS certification (AHA) required.
• Familiarity with EMR systems, specifically Point Click.
