Schedule

Shift: 8:00 AM - 5:00 PM with every other weekend rotation or a permanent schedule of Tuesday-Saturday or Sunday-Thursday, based on agency needs.

Job Description

We are seeking a dedicated and experienced Home Care Registered Nurse (RN) to provide quality care to patients in their homes. The RN will manage a caseload of patients, ensuring their health and wellbeing through effective nursing care and treatment plans.

Responsibilities

• Manage a caseload of 20-25 patients with an expectation of 5-6 visits per day.
• Provide nursing care and perform assessments in the home setting.
• Administer IV therapy, tube feeds, and care for trachs, compression wraps, and other medical treatments as required.
• Utilize EMR (EPIC) for documentation and patient management.
• Collaborate with the healthcare team to address patient needs and improve workflow.

Required Experience / Certifications / Licensure

• Minimum of 2 years of experience as a Registered Nurse.
• Valid BLS certification.
• Experience with Epic and Oasis documentation is preferred.
• Demonstrated skills in IV therapy, wound care, and managing complex medical conditions.
