Job Description

We are seeking a dedicated Physical Therapist with a graduate degree from an accredited Physical Therapy Program. The ideal candidate will have a minimum of one year of physical therapy experience, preferably in home health or a facility setting. The Physical Therapist will be responsible for providing high-quality patient care and support.

Responsibilities

• Provide physical therapy services to patients in a home health setting.
• Assess patient conditions and develop effective treatment plans.
• Document patient progress and adjust treatment plans as necessary.
• Collaborate with interdisciplinary teams for comprehensive patient care.

Required Experience / Certifications / Licensure

• Current and unrestricted Physical Therapy Licensure.
• Current CPR certification.
• HomeCare HomeBase experience preferred.
• Two supervisory references required.
• Travel experience required.
