Licensed Practical Nurse (LPN) - Full Time - New York

Schedule

This is a Full Time position with a shift of 3x12 hours from 7:00 AM to 7:30 PM, scheduled as needed by the client, requiring every other weekend and holiday.

Job Description

The Licensed Practical Nurse (LPN) is responsible for providing direct and indirect nursing care to patients by following defined policies and procedures under the supervision of a Registered Nurse. The LPN must possess the necessary knowledge and skills to cater to the specific needs of adult and geriatric patients. This role involves continuous data collection and communication with the healthcare team to ensure comprehensive resident care.

Responsibilities

• Collect ongoing data relevant to the resident's physical and psychosocial condition.
• Communicate findings to the Registered Nurse and interdisciplinary team members.
• Contribute to the development of culturally relevant care plans that achieve desired outcomes.
• Evaluate resident outcomes and recommend revisions to the care plan as necessary.
• Participate in mentoring and orienting new staff members.
• Maintain knowledge of current nursing practices through continuing education.

Required Experience / Certifications / Licensure

• Current New York LPN Licensure.
• Basic Life Support (BLS) certification through AHA or RQI.
• Minimum of 1 year experience as an LPN in a Long-Term Care (LTC) facility.
