Part time Day and Evening shifts, scheduled as needed by the facility (generally 2x8s/3x8s) + Weekend/Holiday required.

Job Description

The Licensed Practical Nurse (LPN) must demonstrate the knowledge and skills necessary to provide age-appropriate care to patients in the geriatric population. The individual must possess the ability to assess data reflective of the patient's status and interpret the information needed to identify each patient's requirements relative to their age-specific needs.

Responsibilities

• Collect data relevant to residents' physical and psychosocial condition on an ongoing basis and communicate findings to the Registered Nurse and interdisciplinary team.
• Contribute to the development of a culturally relevant plan of care reflecting nursing diagnosis and interventions to achieve desired resident outcomes.
• Recommend revisions to the plan of care based on evaluation of resident outcomes and indicate the need for referrals.
• Collaborate with residents/families and members of the interdisciplinary team to assure customer satisfaction.
• Maintain current knowledge and competency in nursing practice and participate in mentoring/orientation of staff.
• Demonstrate knowledge and competency while considering safety, cost, and impact in planning care delivery.

Required Experience / Certifications / Licensure

• New York LPN Licensure
• BLS certification (AHA or RQI specifically)
• Experience serving in an LPN capacity at a long-term care facility (minimum 1 year required)
