RN Inpatient Care Manager

Job Description

The RN Inpatient Care Manager coordinates the care and services of selected member populations to facilitate a safe and timely transition to the next appropriate level of care. Clinically assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's health needs. The position promotes effective utilization and monitoring of health care resources, ensuring that all aspects of the patient's clinical, psychosocial, and financial needs are addressed for a timely progression of care.

Responsibilities

• Develops and maintains a comprehensive case management plan based on clinical findings and patient assessments.
• Identifies high-risk patients and refers them to social services when necessary.
• Implements actions to prevent readmissions and evaluates causes of any readmissions for future prevention strategies.
• Coordinates and documents the transition of care plan, modifying it as needed to meet patient needs.
• Communicates financial management issues to the healthcare team and patients as necessary.
• Works with the clinical care team to monitor length of stay and proposes solutions to discharge barriers.
• Facilitates seamless transitions of care by actively communicating with patients, families, and care team members.
• Stays updated on federal, state, and regulatory agency standards including The Joint Commission and CMS regulations.
• Educates patients and their families on care progression, service utilization, and safe transitions.
• Identifies potential risk management and quality issues, working collaboratively to resolve them.

Required Experience / Certifications / Licensure

• Excellent communication, negotiation, and conflict resolution skills.
• Data and computer skills with knowledge of relevant regulatory standards related to Case Management.
• Prior clinical experience in the care and management of hospitalized patients is required.
• Experience in acute care case management and training from a professional Case Management organization is preferred.

Schedule

This position is full-time, with a shift from 8:00 AM to 4:00 PM, and no weekend or on-call responsibilities.
