Schedule

Monday to Friday, 8:00 AM to 5:00 PM with a 30-minute lunch break. No call required, with occasional weekend coverage as needed.

Job Description

We are seeking a dedicated RN specializing in Case Management to join our dynamic team. The successful candidate will work primarily on care coordination and discharge planning for our patient population.

Responsibilities

• Care coordination for patients across various departments.
• Facilitate discharge planning to ensure smooth transitions.
• Collaborate with multidisciplinary teams to optimize patient outcomes.

Required Experience / Certifications / Licensure

• Registered Nurse (RN) License.
• Experience in case management, preferably with HCA systems.
• Familiarity with EMR systems such as Midas, Meditech, Interqual, and Care Port.
