Job Description

We are seeking a skilled RN Case Manager to join our team for a 13-week contract. The primary focus of this role will be on discharge planning and effective case management in a fast-paced environment. The RN Case Manager will be responsible for collaborating with medical staff to ensure the best care plans are in place for patients.

Responsibilities

• Conduct discharge planning and maintain progression of care.
• Engage in difficult conversations with patients and families regarding care plans.
• Collaborate effectively with multidisciplinary teams, including physicians.

Required Experience / Certifications / Licensure

• Must have at least 1 year of bedside case management experience.
• Knowledge of post-acute care processes is essential.
• Experience with Meditech or HCA systems is required.
