Job Description

We are seeking a dedicated RN Case Manager for our Home Health division. The ideal candidate will manage patient care for the South team, ensuring high-quality service and support to patients in their homes.

Responsibilities

• Manage a caseload of 5-6 patients per day.
• Conduct patient assessments and develop care plans in accordance with best practices.
• Document all patient interactions in a timely and accurate manner.
• Coordinate care with other healthcare professionals and support staff.

Required Experience / Certifications / Licensure

• 2+ years of experience in home health care.
• Experience with case management and OASIS documentation.
• Wound care experience preferred.
