Job Description

The Licensed Practical Nurse (LPN) will perform direct and indirect resident care under the direction of licensed nursing personnel. This role requires knowledge and skills necessary to provide care appropriate to the age of the patients served, including adult and geriatric populations.

Responsibilities

• Meet residents' Activities of Daily Living (ADLs) as outlined in their care plans, including hygiene, grooming, and incontinence care.
• Maintain resident safety and environment, responding to call lights and alarms promptly.
• Ensure residents' nutritional needs are met and that their preferences are honored.
• Provide rehabilitative care as outlined, including range of motion and proper body alignment.
• Report residents' rights, concerns, and changes in physical or emotional status in a timely manner.
• Understand and follow protocols related to occupational exposure to bloodborne pathogens.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure.
• BLS certification (AHA or RQI only).
• One year of experience in a Long-Term Care (LTC) or Certified Nursing Assistant (CNA) capacity.
• High School Diploma or GED.
