Licensed Practical Nurse (LPN) - Full Time, Nights and Weekends

This is a 13-week contract position.

Schedule

Full-time nights, 11:00 PM - 7:30 AM, with mandatory shifts every weekend and potential for holiday work.

Job Description

The LPN will provide direct and indirect skilled nursing care for residents in accordance with their age-specific needs. The individual is expected to contribute to nursing diagnoses and interventions to achieve desired outcomes by collaborating with residents, families, and the healthcare team.

Responsibilities

• Collect data relevant to residents' physical and psychosocial condition and communicate findings to the RN and other team members.
• Assist in developing culturally relevant care plans with involvement from residents/families and the interdisciplinary team.
• Evaluate resident outcomes and recommend revisions to the care plan as needed.
• Participate in the mentoring and orientation of new staff.
• Maintain compliance with safety and clinical protocols, including effective communication across healthcare disciplines.

Required Experience / Certifications / Licensure

• Active New York LPN Licensure.
• Current BLS certification (AHA or RQI).
• Minimum of one year of experience in a long-term care facility as an LPN.
