Registered Nurse - Case Management

Day shift, 8-hour shifts from 08:00 AM to 04:00 PM.

Job Description

We are seeking a Registered Nurse with extensive Case Management experience to join our team. The ideal candidate will have a background in Acute Care case management and will be proficient in using Cerner. This role involves managing a caseload effectively within a supportive healthcare environment.

Responsibilities

• Manage cases effectively within the hospital setting.
• Coordinate care and services for patients to ensure optimal outcomes.
• Collaborate closely with interdisciplinary teams including Physical Therapy, Social Services, and Pharmacy.
• Utilize Cerner and other relevant systems for patient management and documentation.

Required Experience / Certifications / Licensure

• Minimum 5 years of hospital-based case management experience.
• Previous experience with Cerner/Power Chart and Wellsky (formerly naviHealth) is required.
• PAE knowledge and PASRR entry experience are strongly preferred.
• First-timers are accepted for this position.
• No specific certifications are required.
